I hereby request a distance consultation with author Mark McConville, Ph.D. regarding the content of his book Failure to Launch: Why Your Twentysomething Hasn’t Grown Up … and What to Do About It. I understand, acknowledge and agree that any such consultation does not constitute psychotherapeutic treatment. I further acknowledge and agree that this consultation does not form a doctor-patient relationship, and that any recommendations arising from author’s work or this consultation do not constitute medical or clinical psychological advice. I represent that I understand that should any question arise relating in any way to mental, emotional or behavioral health issues, this consultation is not a substitute for therapeutic care, psychological care, psychiatric care or behavioral health care.
